Can the McKenzie Method Provide Rapid Resolution for Chronic Shoulder Pain Classified as Derangement?

Derangement is one of the syndrome in MDT classification which has a clinical presentation of Directional Preferene (either bending forward, backward etc) in response to loading stategies (lying, standing, therapist generated pressure etc) and is typically associated with a movment loss of the affected area. A common feature observed in the spine is Centralisation of pain.

  1. Assessment and Classification:

    Conducting a thorough assessment enables the clinician to identify the specific type of shoulder pain, such as Derangement, Dysfunction, Postural, or OTHER. This content will primarily concentrate on Derangement syndrome. Upon confirmation, patients typically achieve pain relief within just 3-4 visits.


  2. Exercise Prescriptions:

    Depending on the assessment results, specific direction of repeated movements are suggested to alleviate pain and improve proper functionality. For instance, in Shouder Pain (42% Derangement), 67% were extension or medial rotation responders. That means once assessed patient with shoulder pain would see significant decrease in their pain from following these exercises; HBB hand behing back or shoulder extension exerises to be done at home atleast every 2-3 waking hours, 10-15 reps of 2-3 sets.

  3. Education:

    Patients receive information about their condition and the advantages of self-care. This knowledge enables them to assume responsibility for their healing process and may reduce the occurrence of future pain episodes. MDT places a strong emphasis on patients taking control, with the primary goal of educating them being to ensure they comprehend the nature of pain, its causes, and to empower them to make informed decisions on managing pain through proper repetitive movements.


The strength of the McKenzie method lies in its personalized approach, which adapts to individual mechanics and how patients respond to treatment.


Rapid Resolution: A Case Study


A 63-year-old man came to the clinic with a 3-month history of pain in his right front shoulder after falling on his right side. He was healthy and had never had shoulder issues before. He enjoyed fishing and gardening but couldn't do either activity after the fall. The pain was sporadic but could persist when resting after using his arm. Certain movements like bringing his shoulder across his body or lying on his right side worsened the symptoms. Resting the right shoulder helped alleviate the pain. No X-rays were performed. He had no neck pain or tingling in his arm. His QuickDASH score was 34% at the first visit. He had only used ibuprofen for pain relief. He also tried other treatment such as massage and accupunture without any success.


EXAMINATION:

He exhibited significant restriction in internal rotation (IR) of the right shoulder, experiencing pain at the end of the range of motion, and a minor limitation in shoulder flexion, extension, and abduction, all accompanied by pain during movement. Strength assessment indicated 3/5 strength in internal rotators and 3+/5 in deltoids, both movements causing a pain level of 8/10. By performing repeated internal rotation of the right shoulder, the pain reduced to 3/10, and the range of motion increased by 20°. Strength in internal rotators and deltoids improved to 4/5. A diagnosis of shoulder derangement with a preference for internal rotation was established due to the swift changes in symptoms and range of motion observed during repeated movement assessments.


INTERVATION:

At home, he was instructed to do IR exercises 10-15 times every 2-3 hours while awake. When he came back after 2 days, he reported a 75% improvement. He was advised to do the exercise to endrange with overpressure every 3 hours and to contact if the movement made the symptom worse. The patient was discharged a week later after three visits, with a QuickDASH score of 4%, full range of motion, and normal strength.
